Authorization and Consent
I/We hereby give permission for my child listed on this registration form
To attend the activities of Lucknow Vacation Bible School 2024.
I/WE DO HEREBY RELEASE THE LUCKNOW CCRC and the directors thereof, along with any paid staff or volunteers associated with the Lucknow VBS Ministries from any and all liability, claims or demands for personal injury, sickness, or death, as well as property damage that occur when participating in the participating in the activities of the group.
FURTHERMORE I/WE HEREBY GRANT my/our permission for my child listed on this registration form to fully participate fully in all activities, (with the exceptions or restrictions being emailed to lucknowvbs@gmail.com) and do hereby give permission to take the participant to a doctor or hospital and hereby authorize medical treatment, including but not limited to surgery or medical treatment and assume the responsibility of all medical bills, if any.
FURTHER, should it be necessary for the participant to return home due to medical reasons, disciplinary action or otherwise, we hereby assume all the transportations costs.
I/WE give permission to the Lucknow CCRC to use names, photographs, whether still or in motion, video recordings, and/or recordings of the voice of the participant listed below within the Lucknow CCRC for only non-commercial, legal purposes such as posters, wall display, recorded song, church newsletter, or video/PowerPoint presentation.
